We want to hear about your experiences of health & social care ( e.g., hospital visits, trying to get help/support for your mental health)

Everything you tell us is confidential and we won’t ask for any personal details. It is up to you if you would like to take part. If you do, your feedback will help the people who make decisions improve health and care services for children and young people in Wakefield District.
